Mental Health Intensive Outpatient is an individualized treatment program providing 12 weeks of structured services designed to help you manage your mental health symptoms and make improvements in your life.

SummitStone Health Partners recognizes that some of us are not able to make it to an office setting for therapy. If medical problems or severe anxiety are rendering you home-bound, you may want to consider Home-Based Adult Services. Being home-bound can leave you feeling alone, isolated, not cared for and not heard.
Our Home-Based team provides:
- Individual therapy: We specialize in working with clients who are adjusting to serious illness, chronic pain and end-of-life issues. We can help you find ways of coping with any number of emotional challenges, including depression and anxiety, unresolved trauma, and grief and loss.
- Case Management: Our experienced case manager will assess your needs, link you to resources in the community, and provide advocacy and follow-up.
Our compassionate team of therapists and our case manager will help you feel listened to and cared about, always putting your needs first.
You need not be an older adult to be seen by our Home-Based Adult team. We work with adults 18 and older.
SummitStone offers a vast array of Group Therapy sessions designed to help you help yourself—with help from others. None of us are alone in this journey, and Group Therapy is a proven way to get the most out of what you put into it, including helping others who may be experiencing similar circumstances.
Child, Adolescent and Family Services
SummitStone can help provide a strong start for your child and encourage social and emotional wellbeing. Research shows that early intervention has a lasting impact on the future wellness of your child and family.
Functional Family Therapy (FFT)
FFT is an evidence-based, in-home model that works with the entire family to improve relationships with children 8 to18 who are at risk for or presenting with delinquency, violence, school issues, substance use or mood disorders.
Family Care Coordinator (FCC)
Customized intensive, in-home family therapy serving children 3 to 17 who are at-risk of out-of-home placement due to behavior issues and/or mental illness. Therapists visit the home once or twice a week for six months and focus heavily on caregiver support and skill-building.
